1) If you are a new vendor with Healthy Kids' Catalog, you must print out and complete the following Wholesale Drop-Ship Vendor application package documents:

• Vendor Profile/Agreement Form
• NAFTA Certificate of Origin
• If available, also provide a current copy of your Product Liability Certificate of Insurance.

All four documents are required to set up an account for your company, although the Product Liability Certificate of Insurance requirement may be waived on a case-by-case basis.

If your product is accepted by Healthy Kids' Catalog, you may be requested to provide a Certificate of Insurance that names Healthy Kids' Catalog as an additional insured via a broad-form Vendors Liability Endorsement. If you have questions, please call Merilee Kern at (858) 577-0206.

2) If you are an existing vendor with Healthy Kids' Catalog and would like to submit a new item for review or update information for an existing item, you must Print out and complete the following Healthy Kids' Catalog new product review package documents:

• New Product Proposal Form
• NAFTA Certificate of Origin
• When applicable, also provide a Material Safety Data Sheet (M.S.D.S.) for exact product(s) submitted.

These documents and all appropriate safety labeling on your product package are required for product acceptance by Healthy Kids' Catalog.

In both scenarios, please submit your competed forms and one NON-RETURNABLE sample of each proposed product to the following address (Please do not send perishable or live goods. If your product exceeds 25 lbs or is larger than 3 square feet, please send a photograph or videotape with accompanying product literature):
